- All patients must complete digital check in Form
- Patients who are at high risk for spreading COVID-19 will still be asked to reschedule
- Doctor Appointments will be spaced apart
- Optical Services will be provided also by appointment only

- You may request your prescription via MySecureHealthData.com portal, click for instruction
IN OFFICE PICK UP OR SHIP TO YOU:
- We offer in office pick up or shipping is available for your convenience
TELEHEALTH IS AVALIABLE:
- In some cases, a virtual one-on-one office visit is possible. Our doctor uses video technology to care for you without you having to travel to our office. You may request such appointment by visiting Appointment Request.
In order to keep everyone safe , we have limited appointments in order to serve you better. Therefore we ask that you cancel or reschedule your appointment 24 hours before your appointment to avoid cancellation fee.
